The xed-out smiley is the graphic Kurt Cobain sketched for the Nevermind promo rounds in 1991 on DGC Records, and it ran alongside In Utero (1993) and the MTV Unplugged in New York sessions from November 1993 as one of the definitive Nirvana visuals. Cobain fronted the band up to his passing in 1994.
